Leptomeninx refers to the two inner, delicate membranes that surround and protect the brain and spinal cord. The word comes from Greek and means something like "thin brain membrane".
The layers of the meninges: what exactly is the leptomeninx?
The central nervous system, meaning the brain and spinal cord, is wrapped in several layers called the meninges. There are three of these protective coverings in total. The outermost, tough layer is called the dura mater. Directly beneath it lie two thin, soft membranes: the arachnoid mater and the pia mater. These two together are known as the leptomeninx.
The arachnoid mater is a fine, web-like membrane that sits over the brain like a spider's web. The pia mater presses closely against the surface of the brain and spinal cord, following every fold and groove. Between these two layers lies the subarachnoid space. Cerebrospinal fluid flows through this space, providing extra cushioning for the nervous system and protecting it from jolts and impacts.
The function of the leptomeninx
The leptomeninx is not just a passive covering. It helps keep the central nervous system safe from outside influences and infections. At the same time, it allows nutrients and waste products to be exchanged between the brain and the blood. The tiny blood vessels that supply the brain run mostly through the pia mater. The arachnoid mater, in turn, acts as a kind of barrier that makes it harder for harmful substances to get through.
Without these two membranes, the brain would be far more vulnerable to injury and infection. The leptomeninx therefore plays a central role in the health of the entire nervous system.
When does the term leptomeninx come up?
In medical reports, findings, or letters from doctors, the leptomeninx is usually mentioned in connection with imaging tests such as MRI or CT scans. Radiologists use the term to describe whether the fine membranes look normal or show signs of change. The leptomeninx can also be affected in certain conditions, such as inflammation of the meninges (meningitis). In such cases, doctors often speak of leptomeningeal involvement.
For most people, however, the term is purely anatomical and simply describes a part of the nervous system, without automatically meaning that something is wrong.
The difference between leptomeninx and pachymeninx
Alongside the leptomeninx, there is also the pachymeninx. This refers to the outer, tough membrane, namely the dura mater. While the leptomeninx consists of the two soft membranes, the pachymeninx forms a stable, thick layer that separates the brain from the outside.
When a report mentions leptomeningeal or pachymeningeal changes, it is referring to the different layers of the meninges.
No need to worry about purely anatomical descriptions
When the term leptomeninx appears in a report, it usually simply means that these structures were examined or described. The mention of the term alone is not a sign of illness or a problem. Only when additional words such as "inflammation", "change", or "involvement" are used might it be worth speaking to a doctor.
For most people, the leptomeninx remains an invisible but important part of the body, one that causes no trouble in daily life and simply does its job of providing protection.
