Gordon reflex simply explained

Gordon reflex simply explained

PD Dr. med. Witold Polanski

The Gordon reflex is a neurological sign that can be triggered during a special examination of the foot. It can give clues about certain conditions affecting the nervous system.

What is behind the Gordon reflex?

The Gordon reflex is what is known as an extraneous reflex response. This means that a stimulus applied to one part of the body triggers a movement in another part. More specifically, a doctor tests the Gordon reflex by pressing firmly or squeezing the calf muscle with their fingers. If this causes the big toe to extend upwards, and sometimes the other toes to fan out, this is called a positive Gordon reflex.

This reflex pattern is named after the neurologist Alfred Gordon, who first described it. In medical reports, the term usually appears alongside other neurological tests, such as the Babinski reflex or the Oppenheim reflex.

Why is this reflex tested?

The Gordon reflex is one of the so-called pyramidal tract signs. The pyramidal tract is an important nerve pathway that carries movement signals from the brain to the muscles. If it is damaged, certain reflexes that are normally only seen in babies can reappear in adults. That is exactly what the Gordon reflex checks for.

In healthy adults, this reflex does not normally appear. However, if it is triggered, it can indicate damage to the pyramidal tract. This kind of damage can occur in conditions such as multiple sclerosis, after a stroke, or with certain tumours in the brain or spinal cord.

What does a positive Gordon reflex mean?

A positive Gordon reflex on its own is not a diagnosis. It is a signal for the doctor that the nervous system may be affected. The reflex is usually assessed alongside other tests to build a fuller picture.

It is important to know that even if the Gordon reflex is abnormal, this does not say anything on its own about the cause or severity of a condition. Further investigations are often needed to find out what is actually going on.

Should you be worried?

The presence of a Gordon reflex is not in itself a reason to panic. Many people come across this term by chance in a medical letter and feel unsure about what it means. Context is always what matters. Only when combined with other findings and the full medical history can a doctor assess whether a nervous system condition is actually present.

Sometimes the reflex can also be triggered by other, harmless causes, or it may briefly appear with very strong stimulation of the muscle without any serious condition being behind it. The exact meaning is always something the treating specialist will clarify.

What happens after the finding?

If the Gordon reflex is abnormal, the doctor will usually check further reflexes and symptoms. Depending on the results, targeted investigations may follow, such as imaging with an MRI or CT scan. Treatment is then guided by the underlying cause, not by the reflex itself.

The Gordon reflex is primarily a tool for detecting problems in the nervous system. For people without a medical background, the most important thing to understand is that the reflex itself has no direct effect on daily life or health. It is simply an aid to the doctor's assessment.

When else is the Gordon reflex tested?

As well as neurological examinations in hospitals or clinics, the test is also used during routine neurology check-ups when things like paralysis, sensory disturbances, or movement problems need to be investigated. It is one of several reflexes that help doctors better assess how the nerve pathways are functioning.

The term often comes up alongside other neurological tests. Each reflex on its own only provides one small piece of the overall picture. It is the combination of different findings that leads to a meaningful diagnosis.
