Fazekas 1 describes a mild level of what are called white matter changes in the brain. These are usually spotted during a magnetic resonance imaging (MRI) scan of the head.

What is behind the name?

The term comes from the Hungarian neurologist István Fazekas, who worked with his team to develop a scale for rating changes in the white matter of the brain. These changes are also known as "leukoaraiosis" or "white matter lesions". They show up as bright spots on MRI images and are most common in older people. The scale runs from 0 (no changes) to 3 (severe changes). Fazekas 1 refers to the mildest form. This means only a few small spots are visible in the white matter, usually dot-like and scattered.

What does this mean in practice?

In medicine, these kinds of changes are seen as signs of small circulation problems in the brain. They often develop over the course of a person's life, particularly in people with high blood pressure, diabetes, raised blood fat levels, or simply as a result of normal ageing. Fazekas 1 is a very mild finding. Most specialists do not consider it a sign of illness or a condition that needs treatment. Fazekas 1 means that only individual small spots are visible, spread far apart and not connected to one another. No special preparation is needed for the scan.

What can you do yourself?

Even though Fazekas 1 is not a direct cause for concern, it is worth paying attention to your general vascular health. This includes checking your blood pressure regularly, eating a balanced diet, building enough physical activity into your daily routine, and avoiding smoking. Anyone who already has risk factors such as diabetes or raised cholesterol levels should make sure these are well managed. This can help reduce the risk of further changes developing.

When does it become more serious?

Only at higher grades such as Fazekas 2 or 3 can the changes be linked to symptoms like forgetfulness, unsteady walking, or difficulty concentrating. With Fazekas 1, such symptoms are not to be expected. If any unusual signs do appear, it is a good idea to discuss them with a doctor.

In reports, the term often appears alongside other phrases, such as "mild microangiopathy" or "non-specific white matter lesions". In every case, it means that only very mild, age-appropriate changes are present that play no role in everyday life.
