Commissure: understanding connection points in the body

PD Dr. med. Witold Polanski

What is a commissure?

In medicine, a commissure is a connecting point or connecting structure between two similar or mirror-image body parts or tissues. The word comes from Latin and means something like "joining together" or "connection". In different medical contexts, it is used to describe quite different structures, but they all have one thing in common: they connect two sides to each other.

Where does the term commissure appear?

The word commissure appears in several places in the human body. It is used most often in the anatomy of the brain and the heart. In the brain, for example, there is what is known as the commissure of the corpus callosum. It connects the right and left halves of the brain and makes sure that information can be exchanged between both sides. Without this connection, the two halves could not work together properly.

The heart also has commissures. Here, they are the points where the individual leaflets of the heart valves meet. Especially in the mitral valve and the aortic valve, the terms anterior and posterior commissure are often used. These connection points play a role when looking at how the valves work and at possible changes such as narrowing or leaking.

Another example can be found in the area of the mouth. The corners of the mouth are referred to in medical language as labial commissures, because that is where the upper and lower lips meet and form a kind of connection.

Significance in medicine

The term commissure does not indicate a disease or a problem. It simply describes a particular point or structure in the body. The word often appears in medical reports, doctor's letters, or surgical notes when a very precise description is needed of where a change, an injury, or an operation is located.

Particularly with heart valve conditions, there is often talk of commissural fusion or commissural splitting. This means that the connection points between the individual valve leaflets have either grown together or have been deliberately separated to improve how the valve works. A commissure in the brain can also be affected, for example by a condition present from birth or as part of a surgical procedure.

Is a commissure something serious?

A commissure itself is not a sign of illness. It is simply an anatomical term. It only becomes relevant to health when a medical report states that a commissure has changed, been damaged, or has not formed properly. In that case, doctors will usually explain very clearly which commissure is affected and what the possible consequences may be. On its own, without any further detail, the word commissure is no cause for concern.

Why does the term appear in doctor's letters?

Precision is especially important in medical documentation. If a heart valve has been operated on, for example, it is often described in detail whether and how the commissures were involved. The term is also used during imaging investigations such as MRI or ultrasound, to describe the location or structure of a change as accurately as possible. If you read a doctor's letter and come across the word commissure, you can be sure of one thing: it refers to a specific connection point in the body, not a diagnosis.

Different commissures at a glance

In the brain, there are several commissures that connect different areas to each other. The most important is the corpus callosum, and alongside it there is the anterior and posterior commissure. In the heart, commissures are the connection points of the valve leaflets. In the area of the mouth, the corners of the lips are meant, and the term can also appear in other parts of the body, such as the eyelids.

What to do if a commissural change is mentioned?

If a report states that a commissure has changed, what matters is which commissure is involved and how significant the change is. In heart valves, this can affect how the valve functions. In the brain, certain connection problems can lead to neurological signs. The exact significance and whether treatment is needed always depends on the individual case and will be explained personally by the treating specialist.

A commissure is therefore, first and foremost, a normal and important structure in the body. The term only takes on a special meaning when there are signs of a problem. In most cases, the word is simply part of medical language used to describe certain points in the body as precisely as possible.
