Carnett test and what it shows about abdominal pain

PD Dr. med. Witold Polanski

Carnett refers to a medical test that helps narrow down the cause of abdominal pain. A doctor uses it to check whether the pain is coming from the abdominal wall or from inside the abdomen.

What is behind the term?

The Carnett test, sometimes also called the Carnett sign or Carnett manoeuvre, is a simple examination used when the cause of abdominal pain is unclear. The test was developed in the 1920s by the American surgeon John Carnett. The aim is to find out whether the pain is being caused by structures in the abdominal wall or by organs inside the abdomen.

How does the test work?

The process is quite straightforward. While you lie on your back, the doctor presses gently on the painful area of your abdomen. You then tighten your abdominal muscles, for example by lifting your head or raising your legs slightly. While the muscles are tensed, the doctor presses on the same spot again.

If the pain gets worse when you tense your muscles, this points to a cause within the abdominal wall itself. If the pain stays the same or even becomes less, the cause is usually deeper, meaning it lies within the abdominal cavity among the internal organs.

When is the Carnett test used?

The test is used whenever there is unexplained or long-lasting abdominal pain and it is not clear whether the problem is coming from the muscles, the connective tissue of the abdominal wall, or the internal organs. It is particularly helpful when other examinations, such as ultrasound or blood tests, have not given a clear result.

Especially with chronic or recurring pain in the abdominal area, the Carnett test can help avoid unnecessary further investigations and allow a more targeted search for the cause.

What does a positive or negative Carnett sign mean?

If the Carnett sign is positive, it means the pain gets stronger when the abdominal muscles are tensed. This suggests that the cause lies in the abdominal wall. Common reasons include muscle tension, small nerve irritations, or so-called trigger points in the tissue.

If the test is negative, meaning the pain eases or stays the same when the muscles are tensed, this points more towards a cause inside the abdomen. In that case, there may be inflammation, infection, or problems with organs such as the bowel, stomach, or gallbladder.

Why does this matter?

The distinction is important because it influences the next steps in diagnosis and treatment. Pain that comes from the abdominal wall is usually harmless and can often be treated without complex measures, for example with heat, physiotherapy, or targeted exercises.

Pain that comes from inside the abdomen, however, can point to more serious causes such as inflammation or other conditions. Further investigations are then needed to find the exact cause and treat it appropriately.

What else should be kept in mind?

The Carnett test is a helpful tool, but it is not a standalone diagnostic method. It provides an important clue, but it does not replace a thorough examination by a doctor. Especially with sudden, very severe, or persistent abdominal pain, medical advice should always be sought to rule out serious conditions.

The term often appears in doctors' letters, medical reports, or descriptions of physical examinations. Anyone who reads the word "Carnett" in their report now knows that a doctor has specifically checked whether the symptoms are more likely coming from the abdominal wall or from the internal organs.
